Second victim support at the core of severe adverse event investigation

SUMMARY

    When these safety breaks result in death or a life-threatening event for the patient it is considered a serious adverse event (SAE) or sentinel event. In 2009, Scott et_al introduced a more detailed definition of second victims: "A health care provider involved in an unanticipated adverse patient event, medical error, and/or a patient-related injury who become victimized in the sense that the provider is traumatized by the event. Neither information on the unit managers in charge of the first-level care support 7 of 9 or information related to the recovery . . .
